At Skilled Care Pharmacy, we really care about what you have to say.

 

For many years, we have distributed quarterly surveys to administrators, nurses and other “frontline” users of our services and asked them to grade us in key operational areas of product quality, delivery, communication and responsiveness. We have found the comments made on these “report cards” invaluable in helping us improve our service offerings, and many of the suggestions you and others made have led directly to changes in our policies and procedures.

 

Now, as a convenience to you, we are making the same survey available online. Just fill out the form below and click on Submit. It only takes a few minutes, requires no postage or paper and your response and comments will be greatly appreciated by our staff.

 

In addition to our survey of facilities, Skilled Care Pharmacy will continue to offer comment cards at the facility so that families of residents may share their observations or concerns. We will also continue to mail a satisfaction survey bi-annually to residents in each of the facilities we serve.

 

Information from all of these surveys is compiled and summarized, becoming part of each facility’s “care plan” as well as our overall quality assurance program. At Skilled Care Pharmacy, we are striving for continual improvement, and we appreciate your help.
